Dear Funding Applicant,

 

For Augmentative Communication Consultants, Inc. to approach your private health insurance, Medicaid or Medicare, you must submit all of the information needed to process your claim.  This information is listed on the on Funding Checklist.  Please note that information that is incomplete can delay the shipment of your device. 

 

The Funding Checklist is provided to help you track the forms needed to submit your application.  Once you complete all the forms correctly, send them to Augmentative Communication Consultants, Inc. either by fax (412 269-0923) or mail to the above address.  Please be sure to make a copy of all forms for your records. 

 

ACCI will process the claim for you through your private Insurer, Medicare. 

 

Please understand that any co-payment is your responsibility and will be expected to paid in advance of your order.  For Medicaid and Private Insurance, we will need to obtain Authorization or Pre-approval for the device before it can be shipped to you.   Authorization can take anywhere from three to five weeks.
